Arizona Out of County Residence Affidavit

Directions:
1. Indicate the semester you will be attending on the top right side of the form.
(Fall 2009, Spring 2010, Summer I, or II)
2. Complete form with your name, physical address and mailing address. (Must live in Apache County.)
3. Sign affidavit in front of a Notary. (Present a picture I.D.)
4. Indicate which College you will be attending and list Apache County as your county of residence.
5. IMPORTANT! Mail completed and notarized Arizona Out of County Residence Affidavit to:

PO Box 548
St. Johns, AZ 85936
6. Please indicate whether you would like a copy of the affidavit faxed to the college of your choice. Please include the fax number.
7. The college of your choice will submit your affidavit to the State of Arizona for payment of your Out of County tuition.
